You will provide a high standard of clinical assessment treatment and support to clients of the St. Vincents integrated adult MHS and their carers demonstrating excellence in discipline specific and recovery-focused case management skills.
You will join a dynamic community mental health team operating under a recovery-oriented model of care utilising the Strengths Model of Case Management.
Classifications for this position will range from RPN Grade 4 NP75 - NP77 ($112465 - $121113 per annum pro rata)

About the Mental Health Team
The Clarendon Clinic Continuing Care Teams provide ongoing specialist treatment and support to meet the identified needs of clients who suffer with a serious mental illness and associated high level of disability in the City of Yarra. The multidisciplinary team provides a range of clinical and therapeutic interventions using the recovery-focused Strengths Model of Recovery-Oriented Practice.

St. Vincents Mental Health (SVMH) includes an adult area mental health service for the inner city areas of Yarra and Boroondara. The adult mental health service components are: The Footbridge Community Care Unit (CCU) the 44 bed Acute Inpatient Service (AIS) located on the St. Vincents Hospital site the North Fitzroy PARC and two community mental health services (CMHS) at Hawthorn and East Melbourne (Clarendon CMHS). Community mental health services comprise the Triage and Crisis assessment and Treatment (CAT) Service and the Primary Intervention Care Team (PICT) includes Extended Triage and HOPE (Suicide Prevention) Continuing Care (CCT) Mobile Support and Treatment (MST) and Homeless Outreach (CHOPS) services. A Consultation & Liaison Service is provided to the health service. NEXUS Dual Diagnosis Service the Body Image and Eating Disorders Assessment and Treatment Service (BETRS) are regional services. Statewide services are the Victorian Dual Disability Service and the Victorian Transcultural Mental Health. The Acute Inpatient Service includes 5 beds designated as the Koori State-wide Inpatient Service and linked to the Victorian Aboriginal Health Service.
St Vincents Mental Health is committed the recruitment and retention of high quality staff. The service aims to support staff to maintain and develop their skills to provide excellent clinical care and to meet their own personal and career goals. SVMH has a strong education and research focus in addition to affiliations with a number of universities and educational institutions. All SVMH clinical staff are expected to be familiar and practice in a manner consistent with the National Practice Standards for the Mental Health Workforce.
The Clarendon Continuing Care Team provides ongoing specialist treatment and support to meet the identified needs of clients who suffer with a serious mental illness and associated high level of disability in the City of Yarra. Intervention includes a range of clinical and therapeutic interventions using the Strengths model of individual case management.
Under the guidance of the Team Manager and Team Consultant Psychiatrist and in conjunction with team members provide a high standard of clinical treatment and support for a case load of clients presenting with multiple complex problems including serious mental illness associated social relationship issues everyday living skill needs and Dual Diagnosis. The position also provides leadership and supervision functions for nursing and more junior staff actively contributing to quality activities at a local and mental health service addition to the clinical work a significant component of the role involves the development and maintenance of sustainable working partnerships with a range of other service providers both internal and external of the St Vincents Health organisation.
St Vincents Mental Health utilises the Strengths model of case management the position will actively contribute to the implementation of the Strengths model at the Clarendon site and across the wider mental health service.
