Developmental Pathways (DP)is seeking aSenior Case Manager (SCM) Active and Coveragein ourCase Management Department.The Senior Case Manager will manage caseloads in coverage to ensure support is provided aligning with rules and regulations using appropriate delegation. The Senior Case Manager will provide training leadership and mentoring for Active and Coverage Case Managers. They are responsible for verifying eligibility and developing monitoring and assuring implementation of the Service Plan (SP). The Senior Case Manager will ensure that services and supports identified in the annual service plan are coordinated among various providers for continuity of services and funding for individuals served.
Our ideal Senior Case Manager Active and Coverage will communicate in appropriate effective and respectful manner with individuals served supervisors other staff agencies and organizations; maintain individuals served confidentiality. Also will maintain a professional and ethical manner in all interactions and meet performance quality customer service and coordination standards as assigned by department leadership.
This role offers a 4-day 40-hour workweek with flexible scheduling options of either Monday-Thursday or Tuesday-Friday. The role includes the flexibility to work remotely as well as the opportunity to collaborate in-person at our headquarters office in Aurora CO and/or at off-site locations.

Five (5) years of experience in the field of Long-Term Services and Supports
(LTSS) which includes Developmental Disabilities; or
Some combination of education and relevant experience appropriate to therequirements of the position.
Must have demonstrated exemplary performance while working within service coordination or case management positions including the following areas of performance: demonstrated initiative within the department or agency on projects demonstrated leadership skills adaptation to change and problem-solving skills
Valid Colorado motor vehicle license and provide proof of motor vehicle insurance
Meet our driving requirements - no suspensions and no more than two (2) tickets in the past three (3) years
Upon request qualifying need and approval driving requirements may be waived in lieu of reliable transportation.
Preferred
Knowledge of rules and regulations for Health Care Policy and Financing and Colorado Department of Public Health and Environment
Two (2) years experiencein service coordination/case management at Developmental Pathways or similar agency
Familiar with Microsoft Office (Excel Word Outlook etc.) and be able to use these and other required programs at an intermediate level
Additional Responsibilities
Serve as subject matter expert for applicable programs and services; serves as resources/point of contact for complex cases
Complete various assessments to determine eligibility for services/supports and identify individual strengths needs concerns and preferences through interviewing observing and utilization of standardized tools
Provide care coordination services and interventions by referring educating collaborating and meditating with the individual and external providers/agencies
Complete accurate case documentation in appropriate tracking and databases; prioritize workload and meet all required timelines
Complete required documentation related to monitoring activities and other targeted case management activities as PMPM; evaluate need and effectiveness of services requested and delivered; advocate for individual service needs
Schedule facilitate document and complete follow-up for all required meetings; disseminate updates and obstacles to the necessary parties
Ensure the accuracy and timely entry and distribution of required documentation to all necessary parties including but not limited to providers individuals and family members
Maintain knowledge of state/federal regulations policies and procedures regarding long-term services and supports Medicaid waiver and state-funded programs and other community resources
Provide oversight and coaching to case management level staff regarding support for people in services conflict resolution and negotiating positive outcomes for the Department and people in services
Supports and mentors colleagues in the areas of core competencies and specialty areas related to their job tasks as a Case Manager
Shares responsibility of shadowing any meeting for new staff
Participates in facilitating training opportunities to enhance the knowledge and skills of other CMs
Available to provide technical assistance on a weekly basis to other case managers
Review allocations with PM to determine the appropriate level of funding for each individual
