Sirv is seeking experienced Disability Support Workers to provide high-intensity daily personal activities and mental health supports for participants with complex needs. Youll deliver safe person-centred care in home and community settings working within NDIS Practice Standards and Sirvs clinical governance and risk frameworks.
Key Responsibilities
- Deliver high-intensity supports (e.g. complex personal care mealtime management pressure care continence care) in line with participant plans and clinician guidance.
- Provide trauma-informed mental health support including proactive de-escalation emotional regulation strategies and recovery-oriented practice.
- Implement strategies from allied health and behaviour support plans to promote safety skill development and community participation.
- Administer and prompt medication per policy; complete accurate documentation progress notes incident reports and risk alerts.
- Use hoists and other assistive technology safely; maintain infection prevention and a clean dignified care environment.
- Communicate clearly with participants families/guardians clinicians and the Sirv leadership team; escalate risks promptly.
- Uphold participant rights choice and control cultural safety and safeguarding obligations.
Essential Criteria
- Demonstrated experience in high intensity support and mental health support.
- Certificate III (or higher) in Individual Support Community Services Mental Health Nursing or related field.
- Manual Handling: current certificate or willingness to obtain prior to commencement (required).
- Strong understanding of person-centred trauma-informed and recovery-oriented practice.
- Competent with documentation digital apps and following clinical/behaviour plans.
- Availability for a mix of shifts (days evenings sleepovers weekends public holidays).
- Current NDIS Worker Screening Check (or ability to obtain) and Working with Children Check/Blue Card (or ability to obtain).
- Current First Aid & CPR (or willingness to obtain before starting).
- Valid driver licence and reliable insured vehicle (or reliable transport).
