Overview


Catholic Charities of Buffalo is seeking a dedicated part-time Wellness Nurse to join a preventive program that supports older adults through advocacy wellness services and community-based care to help them maintain independence and quality of life. The Nurse is a licensed nurse (RN or LPN) responsible for visiting clients in their homes supporting the physical emotional and social well-being of this older adult population enrolled in the program a preventive care initiative. This position promotes person-centered wellness through health monitoring educational programming and coordination with an interdisciplinary team. The role focuses on preventive and holistic approaches that enable aging in place and optimize quality of life.

The typical schedule for this position will be Monday Tuesday Wednesday and Friday 8:45am - 1:45pm.

Essential Functions

  • Promote active engagement in health improvement and self-care among older adults

  • Educate clients on preventive care nutrition physical activity and emotional well-being

  • Conduct routine health screenings including blood pressure and weight monitoring

  • Monitor and document vital signs and other health indicators regularly

  • Track client progress and communicate significant health changes to supervising clinicians or physicians

  • Maintain accurate confidential records of client interactions and wellness metrics

  • Support clients in understanding and managing their prescribed self-care routines

  • Create personalized wellness plans aligned with individual goals preferences and capabilities

  • Design and lead group classes focused on fitness stress reduction and resilience

  • Demonstrate safe adaptive exercise techniques suitable for older adults

  • Organize and facilitate health promotion seminars and community wellness events

  • Coordinate program scheduling and staffing in collaboration with supervisors

  • Assess and recommend improvements to wellness programs based on client feedback and outcomes

  • Develop new initiatives responsive to evolving client needs and interests

  • Facilitate therapeutic and creative expression activities including mindfulness and relaxation

  • Support access to mental health services and help clients navigate barriers to care

  • Encourage spiritual reflection and personal practices in alignment with client values

  • Lead legacy-based programs (e.g. life storytelling creative journaling) to foster connection

  • Build and maintain positive relationships with clients and their families

  • Promote peer engagement volunteerism and community involvement


Minimum Qualifications

  • Active New York State license as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) required

  • Additional education in Gerontology Public Health or Social Work is desirable

  • Supplemental training or certification in wellness coaching gerontology or holistic health is an asset

  • Minimum of 2 years experience working with older adults or coordinating wellness and community-based programs

  • Strong understanding of aging-related issues with emphasis on aging in place

  • Demonstrated ability to conduct assessments and create personalized wellness plans

  • Excellent interpersonal communication and organizational skills

  • Proficiency in Microsoft Office and comfort using telehealth or assistive technology platforms

  • Cultural competence and sensitivity to working with diverse populations


Physical Requirements

  • Ability to stand walk or move throughout the day to lead activities and engage with clients

  • Capability to lift and carry light equipment or supplies (up to 25 pounds) as needed for wellness classes and events

  • Comfortable bending stretching and demonstrating physical movements during exercise or fitness instruction

  • May occasionally require setting up or rearranging spaces for group activities which involves light physical exertion

Work Environment

  • Standard office and community-based settings including client homes and program sites

  • Frequent local travel within service area

  • Regular physical activity involved such as demonstrating light exercise movements or wellness techniques

  • Occasional lifting of light equipment or supplies (up to 25 lbs) for classes or events

  • May include standing walking or facilitating movement-based sessions for extended periods

  • Exposure to various home environments; adherence to safety and health protocols

  • Hybrid engagement may occur using telehealth or virtual platforms
