The Intensive Case Manager provides services to identified cases assisting with stabilization promoting self-sufficiency and long-term integration for clients and/or family members. Services will be provided to special populations that may include the following: complex medical needs refugees living with mental health issues including those with acute psychological distress related to PTSD clients who have spent unusually long periods of time in refugee camps single mothers elderly refugees without family support systems unaccompanied minors LGBTI-identified refugees HIV-positive and other refugee populations with intense medical mental health or social integration issues.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Ensures intakes and assessments are provided in compliance with program protocols; including providing a mental health screen training staff and interpreters as necessary.
  • Initiates service plans for all eligible clients reviews and closing cases in a timely manner and in compliance with program protocols.
  • Provide solution-focused counseling and crisis intervention support as needed to clients.
  • Provides trained mentors to at least 50% of the eligible client population maintains data collection and documentation of mentorship activities.
  • Provides Enhanced Cultural Orientation based on program protocols to assist special populations in their integration process into the United States.
  • Serves as an advocate and resource for refugee clients and provides equal treatment to all client groups in accordance with the agencys commitment to the dignity of all persons.
  • Works in collaboration with New Mexico refugee resettlement staff to develop training plans and protocols for working with high needs to improve referral processes and increase staff sensitivity and knowledge.
  • Participates in community-wide events to inform the target population about resources and services. Organizes groups community and one-on-one meetings to provide education activities. Assist individuals/families with referrals and access to mainstream community services.
  • Maintain client case notes of all contacts with the client and document all client services in a timely and accurate manner. Maintaining high quality case file documentation.
  • Provide data required for various reports; submit all required reports in a timely and accurate fashion.
  • Ensure that program services are provided in accordance with quality assurance standards of the agency state accrediting organizations and HIPAA.
  • Comply with all federal state and agency contracts.
  • Perform other duties as assigned.

QUALIFICATIONS

  • Broad knowledge of practices principles and methods employed in the resettlement of refugees including a working knowledge and understanding of U.S. Immigration laws and regulations.
  • Has an understanding of local mainstream community resources and how to access resources.
  • Cross-cultural sensitivity and knowledge with the ability and desire to work with people of other cultures.
  • Outstanding organizational ability skills in problem solving negotiations decision making and mediating conflict. Computer skills in Word Excel and the use of email and other applications written communication skills ability to edit and the ability to make presentations.
  • Ability to explain facts and information to clients for who English is not a primary language.
  • Maintains punctuality for work appointments and report dates.
  • Possesses a valid New Mexico drivers license; has a reliable and consistently available vehicle; and is able to regularly travel throughout the agencys service area by way of interstate highways and primary and secondary roads. Must carry automobile liability insurance at the level of 100000/300000/100000.

EDUCATION and/or EXPERIENCE

Bachelors degree or equivalent experience with a focus in Social Work Humanitarian Counseling Psychology or other related field required. Spanish fluency preferred.
