Job Title: VT - Home Health RN Case Manager
Job Location: Barre VT

Job Description:

Key Responsibilities:

Perform in-home patient assessments and develop individualized care plans

Coordinate care among interdisciplinary teams including physicians therapists and social workers

Monitor and document patient progress and update care plans accordingly

Educate patients and families on treatment plans medications and health maintenance

Ensure compliance with all regulatory guidelines and standards

Qualifications:

Current and valid RN license in Vermont (or compact state)

Minimum of 1 year of recent experience in Home Health or Case Management

Proficiency with OASIS documentation preferred

Strong communication organizational and time management skills

Valid drivers license and reliable transportation
