Position Summary:
The Patient Navigator is responsible for supporting patients by serving as the primary liaison between patients providers and office staff. This role includes managing calls routed from the call center assisting patients with scheduling and rescheduling appointments addressing patient inquiries and coordinating necessary follow-up. The Patient Navigator ensures a positive patient experience by maintaining excellent communication supporting access to ancillary services and upholding the highest standards of confidentiality and security of patient information at all times.
Essential Functions & Job Responsibilities:
Schedule and coordinate patient appointments including rescheduling when necessary.
Facilitate a smooth patient experience during onsite visits and when engaging with ancillary services.
Serve as a liaison between patients and providers ensuring clear communication and resolution of patient needs.
Manage and navigate calls coming through to the office from the call center.
Conduct outreach to patients who may be due for follow-up appointments procedures or other care needs.
Collaborate with site leadership to help resolve patient concerns in a timely and professional manner.
Assist with translation needs including coordination with language line services.
Ensure patient confidentiality and security of protected health information in compliance with HIPAA and organizational policies.
GENERAL QUALIFICATION STANDARDS & JOB REQUIREMENTS:
Education/Certifications/Licenses:
High School Diploma or equivalent
Performance Standards for Specialty Knowledge Skills & Abilities:
Working knowledge of basic computer programs and excellent typing skills.
Ability to plan prioritizes and completes delegated tasks in a reasonable span of time.
Ability to work independently and as a member of a team.
This position may require cross-coverage of other sites as needed and therefore a measure of flexibility to respond adequately to changing demands is .
Superior customer service skills.
Excellent communication skills both verbal and written.

The provided compensation range is based on industry standards and salary determinations will be made based on numerous factors including but not limited to years of experience and location of position.
