Job Summary
Are you a compassionate and dedicated social worker with a passion for making a difference in the lives of patients and their families We are seeking a Medical Social Worker with a Masters degree to join our dedicated hospice team in the beautiful Baker City. This is a meaningful opportunity to provide emotional support guidance and resource coordination for our patients in their most critical times.
Key Responsibilities:
- Provide comprehensive assessments of patients and families to identify psychosocial financial and emotional needs.
- Develop and implement individualized care plans based on patient needs and preferences.
- Offer counseling and support to patients and families dealing with end-of-life issues grief and loss.
- Coordinate with interdisciplinary teams to ensure holistic care and support.
- Assist families in navigating community resources and support systems.
- Advocate for patient and family rights and wishes within the care team.
Qualifications:
- Masters Degree in Social Work from an accredited institution.
- Current licensure as a Social Worker in the state of Oregon.
- Experience in hospice or palliative care is highly preferred.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a collaborative team.
- Strong organizational and time management skills.
