Qualifications:
- Education:Masters Degree in Social Work (MSW)
- Licensure:Active NY State license
- Experience:Minimum of 1 year of experience
- Technical Skills:Proficiency with electronic health records (EHR).
- Soft Skills:Empathy cultural competence and a patient-centered approach.
Key Responsibilities:
- Conduct comprehensive psychosocial assessments and diagnoses for patients and their families.
- Provide evidence-based individual group and family therapy.
- Develop implement and monitor individualized treatment plans in collaboration with a multidisciplinary healthcare team.
- Perform crisis intervention and risk assessments implementing appropriate safety plans as needed.
- Advocate for patients and families to ensure access to community resources social services and financial assistance programs.
- Provide care coordination including patient education discharge planning and referrals to appropriate follow-up care.
- Maintain timely accurate and confidential patient records in compliance with all state federal and organizational policies.
- Participate in case conferences team meetings and ongoing professional development opportunities.
