MIDAS/Meditech Experience PASRR Navihealth

MSW or LCSW

List Typical Procedures Performed on Unit(s):

Discharge planning MDR Referrals Working with Family and Patients

Number of Beds: 344 beds at acute care hospital (excluding BH and acute rehab)

Patient Ratios: SW if pair with Matrix of another RN will work Complex case of 8 or 9. If they are just covering DC planning then will have 1:25 or up to 30

Documentation System: MIDAS/Meditech Nate Navihealth Care Team Assignment

Call Requirements: All work every third weekend

Floating policy: All Social workers float-except trauma.

Patient Experience requirements: Knowledge of Hospital DC planning/execution and knowledge of MDR or IDT
