Requirements:

Education: Graduate of ACEN/CCNE accredited nursing program; foreign grads must hold current unrestricted license.
Licensure: Active unrestricted RN license.
Certification: One or more: CCM CDMS CRRN COHN COHN-S ACCC CRC RN-NCM CMC.
Experience: Minimum 1 year post-licensure experience in case management.
Duties: Care coordination discharge planning interdisciplinary care support for complex/multiple conditions outcome evaluation family support.
