Job Summary

As a key member of our care team the Nurse will ensure that each individual receives quality personalized health care that respects their choices and promotes their dignity. Youll provide care in accordance with Indiana Nursing Licensure Laws and Regulations support the interdisciplinary team and act as a liaison between individuals staff and medical professionals.

Youll play a hands-on role in everything from planning and delivering care to auditing charts and medication management to training direct care staff. Your work will help maintain safe healthy environments and empower individuals to achieve the best possible outcomes.

What You Get To Do:

  • Deliver hands-on person-centered health care that prioritizes dignity respect and quality of life
  • Serve as a trusted health care resource and consultant for staff families and interdisciplinary teams
  • Conduct comprehensive health assessments chart audits and medication reviews to ensure compliance while identifying and addressing emerging health concerns through preventative measures and reporting communicable diseases
  • Respond to on-call needs and health-related emergencies with expert guidance and solutions
  • Coach and support direct care staff promoting best practices in medication administration and daily health routines
  • Act as a liaison with medical professionals and community health partners participating in team meetings and care planning to ensure each individuals needs are met

Qualifications :

MUST HAVE: 

  • Current LPN or RN licensure in Indiana
  • Minimum 2 years experience in providing health care to individuals with disabilities or in long-term care
  • Valid drivers license with an acceptable driving record
  • Proficiency with Microsoft Office Suite (Word Outlook Excel PowerPoint) and health care documentation platforms (Therap Kronos)
  • Ability to be on-call and flexible to meet the needs of individuals staff and homes

NICE TO HAVE: 

  • Certifications: CPR First Aid Medication Administration Trainer
  • Experience in community-based or residential disability services
  • Familiarity with HIPAA/HITECH compliance and healthcare regulations
  • Experience providing training and coaching to direct support staff
  • Prior experience collaborating with interdisciplinary teams
