Job Description:
We are seeking dedicated and compassionate Case Managers to join our team in the Burbank this role you will work directly with adults with intellectual disabilities providing support and connecting them with resources to help them thrive in their communities. This position requires travel to clients homes within the designated area.
Key Responsibilities:

Provide case management services to adults with intellectual disabilities.

Conduct regular home visits to assess client needs and monitor progress.

Develop implement and monitor individualized service plans.
-
Advocate for clients and coordinate with families service providers and community resources.

Maintain accurate and timely documentation and case notes.

Participate in team meetings and trainings as required.

Work occasional weekends as needed.
Work Schedule:
Compensation & Benefits:

Pay: $26.00 $29.00 per hour based on experience

Mileage reimbursement for travel

Requirements:

Must be bilingual in English and at least one of the following languages:

Valid drivers license and reliable transportation.

Ability to work independently and manage time effectively.

Experience working with individuals with intellectual disabilities preferred.
