Family Medicine at Scappoose is a primary care Family Medicine clinic and Rural Health Center (RHC). The patient population includes pregnant people babies children adolescents adults and geriatrics. The patients are generally moderately stable to moderately unstable with complex medical conditions and SDOH barriers that require medical and nursing interventions. These include but are not limited to oral and injectable medications foot care telephone and in-clinic triage care coordination health education complex wound care pain management and behavioral and/or crisis management.
The OHSU Clinical registered nurse (RN) provides compassionate evidence-based and efficient care to individuals families communities and patient populations. The Clinical RNs care delivery is consistent with the Oregon Nurse Practice Act the American Nurses Association (ANA) Scope and Standards of Practice and the ANA Code of Ethics. The Clinical RN demonstrates the professional role obligations of scientist leader practitioner and knowledge transferor Onsomble Model of the Professional Role. Professional accountability enriches the Clinical RNs engagement as a leader in promoting an inter-professional culture of collaborative decision-making innovation life-long learning and teamwork. Nurses perform patient visits telephone triage and advice and manage an in-basket workload in EPIC where pts. are reviewed for Care Management Care Coordination and follow up for hospital and ED discharges.
