We are seeking a compassionate and experienced Clinical Care Partner (RN) to join our team. This role plays a pivotal part in ensuring participants receive personalised safe and high-quality clinical care.
The Role:
As a Clinical Care Partner you will:
- Conduct comprehensive nursing assessments and develop monitor and review individual care plans and healthcare plans for participants across all funding models.
- Provide leadership guidance and clinical support to staff including non-clinical Care Partners and Community Support Professionals.
- Ensure care delivery aligns with participants changing needs clinical risks and goals while promoting culturally appropriate trauma-informed care.
- Collaborate with participants families and multidisciplinary teams to assemble the right care team and coordinate services effectively.
- Engage in ongoing care management education advocacy and support to empower participants and enhance their overall wellbeing.
Key Responsibilities:
- Conduct comprehensive clinical assessments and care planning.
- Monitor review and respond to participants changing care needs.
- Provide clinical oversight supervision and training to staff delivering care.
- Coordinate services with internal and external stakeholders ensuring safe and high-quality care.
- Maintain accurate documentation budgets and compliance with program and statutory guidelines.
- Build and maintain professional networks to support participants health and wellbeing.
- Participate in continuous improvement professional development and quality initiatives.
Selection Criteria Essential:
- Current RN registration with AHPRA.
- Evidence of ongoing professional development.
- NDIS Worker Screening Clearance / Criminal Record Check.
- Current Australian Drivers Licence and reliable insured vehicle.
- Experience in case management care coordination or similar roles.
- Knowledge of aged care NDIS and health care systems.
- Experience supervising supporting and training staff.
- Strong communication problem-solving and organisational skills.
- Ability to work flexibly and respond to participants changing needs.
Desirable:
- Experience in service coordination in health/aged care or community services.
- Current First Aid/CPR or advanced life support certifications.
- Availability for on-call support.
- Current Working with Children Check (if required).
