Job Description Summary
The registered nurse plansorganizes and directs hospice care and is experienced in nursing with emphasison community health education/experience. The professional nurse builds fromthe resources of the community to plan and direct services to meet the needs ofindividual and families within their homes and communities.
Essential Job Functions/Responsibilities
Patient Care
1. Completes an initial comprehensive and ongoingcomprehensive assessment of patient and family to determine hospice a complete physical assessment and history of current and previousillness(es).
2. Provides professional nursing care by utilizing allelements of nursing process and as defined in the state Nurse Practice Act.
3. Assesses and evaluates patients status by:
A. Writing and initiating plan of care
B. Regularly re-evaluating patient and family/caregiverneeds
C. Participating in revising the plan of care asnecessary
4. Initiates the plan of care and makes necessaryrevisions as patient status and needs change.
5. Uses health assessment data to determine nursingdiagnosis.
6. Develops a care plan that establishes goals based onnursing diagnosis and incorporates palliative nursing actions. Includes thepatient and the family in the planning process.
7. Initiates appropriate preventive and rehabilitativenursing procedures. Administers medications and treatments as prescribed by thephysician in the physicians plan of care.
8. Counsels the patient and family in meeting nursingand related needs.
9. Provides health care instructions to the patient asappropriate per assessment and plan.
10. Assists the patient with the activities of dailyliving and facilitates the patients efforts toward self-sufficiency andoptional comfort care.
11. Acts as Case Manager when assigned by ClinicalSupervisor and assumes responsibility to coordinate patient care for assignedcaseload.
Communication
1. Completes maintains and submits accurate andrelevant clinical notes regarding patients condition and care given. Recordspain/symptom management changes/outcomes as appropriate.
2. Communicates with the physician regarding thepatients needs and reports changes in the patients condition;obtains/receives physicians orders as required.
3. Communicates with community health related persons tocoordinate the care plan.
4. Teaches the patient and family/caregiver self-caretechniques as appropriate. Provides medication diet and other instructions asordered by the physician and recognizes and utilizes opportunities for healthcounseling with patients and families/caregivers. Works in concert with theinterdisciplinary group.
5. Provides and maintains a safe environment for thepatient.
6. Assists the patient and family/caregiver and otherteam members in providing continuity of care.
7. Works in cooperation with the family/caregiver andhospice interdisciplinary group to meet the emotional needs of the patient andfamily/caregiver.
8. Attends interdisciplinary group meetings.
9. Performs other related duties as assigned to supportthe overall goals and objectives of the organization.
Additional Duties
1. Participates in on-call duties as defined by theon-call policy.
2. Ensures that arrangements for equipment and othernecessary items and services
are available.
3. Supervises ancillary personnel and delegatesresponsibilities when required.
4. Assumes responsibility for personal growth anddevelopment and maintains and upgrades professional knowledge and practiceskills through attendance and participation in continuing education andin-service classes.
5. Fulfills the obligation of requested and/or acceptedcase assignments.
6. Actively participates in quality assessmentperformance improvement teams and activities.
7. Other duties as assigned by supervisor.
Position Qualifications
1. Graduate of an accredited school of nursing. One (1)to two (2) years of recent acute care experience in an institutional settingand one (1) to two (2) years of recent experience in home care related tohospice.
2. Current licensure in State and CPR certification.
3. Must be a licensed driver with an automobile that isinsured in accordance with state and/or organization requirements and is ingood working order.
4. One (1) year of hospice care experience required.
5. Management experience not required. Responsible forsupervising hospice aide/home care attendants.
6. Excellent observation verbal and writtencommunication skills problem solving skills basic math skills; nursing skillsper competency checklist.
7. Prolonged or considerable walking or standing. Ableto lift position and/or transfer patients. Able to lift supplies andequipment. Considerable reaching stooping bending kneeling and/or acuity and hearing to perform required nursing skills.
8. Reliable transportation required.
