Job Summary and Responsibilities
This is a weekend position 24 hours per week. Flexible schedule - either 4-5 shorter days or 2-3 longer days that must include Saturday and Sunday and will be for coverage in the Durago service area
As a Registered Nurse with ChHI Health at Home youll be at the heart of patient care delivering expert clinical services coordinating with multidisciplinary teams and building meaningful relationships with those you serve. Responsibilities:
- Deliver compassionate high-quality nursing care tailored to each patients needs.
- Develop and adjust individualized care plans to ensure optimal patient outcomes.
- Collaborate with physicians therapists and other healthcare professionals.
- Supervise your own caseload nurturing the growth and development of LPNs LVNs and Home Health Aides to facilitate and cultivate a team of caregivers.
- Provide education and support to patients and their families.
- Maintain thorough documentation to meet regulatory and operational standards.
- Utilize the latest technology and best practices to enhance patient care.

Job Requirements
- Graduate of an accredited nursing program; must hold an active RN license in the state served.
- At least 1 year of nursing experience (Home Health preferred).
- Strong critical thinking clinical expertise case management and a passion for patient care.
- CPR/BLS certification
- Valid drivers license
