Schedule: Full-time; Monday-Friday 8am-5pm
About the Role:
Hospice of Marion is seeking a Social Worker to support patients and families during their end-of-life journey. This role provides psychosocial support care coordination and resource assistance to ensure comfort and dignity for those we serve.
Key Responsibilities:
- Assess patient and family needs related to coping mental health financial resources and caregiver support.
- Provide individual and family counseling crisis intervention and bereavement support.
- Collaborate with the Interdisciplinary Group (IDG) to develop and manage patient care plans.
- Assist with financial resources Medicare/Medicaid and long-term care planning.
- Maintain accurate electronic medical records (EMR) and participate in IDG meetings.
- Support patients and families with advance directives and memorial planning.
- Educate the community on hospice care.
- Participate in an on-call rotation.
Qualifications:
- Education/Experience: Bachelors degree in social work or a related field; one year of clinical experience preferred.
Physical Demands:
- Ability to lift up to 25 pounds frequently and up to 250 pounds with equipment.
- Frequent mobility and possible exposure to communicable diseases and various environments.
