Job Description
Hospice of Marion County a member of Empath Health is seeking an LPN to join our team!
M-F 8a to 5p this role visits patients in facilities in Marion County
Under the direction of the Primary RN the LPN is responsible for identifying Patient/Family needs and coordinating Interdisciplinary Team (IDT) provision of services under physicians orders. The LPN utilizes monitoring intervention and teaching skills to maximize patient comfort and enhance the quality of life for the Patient/Family unit. The LPN is supervised and collaborates with Primary RN and/or CTM.
REQUIREMENTS:
- Education and/or Experience: Associates degree (A.A.) or equivalent from two-year college or technical school; or six months to one year related experience and/or training; or equivalent combination of education and experience.
- Licensure:
o Valid Florida LPN License.
o Valid Florida State Drivers License.
- Continuing Education: As required for licensure.
- Certifications:
o Valid CPR certification from the American Red Cross or American Heart Association
o Current IV Therapy certification preferred. Must obtain the certification within six months of hire date.
- Additional experience in other health care settings desirable.
- For field-based positions employees must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance a copy of which will be placed in the employees HR file.
- The ability to type 25 wpm is highly preferred. Must receive a passing grade on a pre-employment computer literacy evaluation.
