Job Summary

About the role
Support children and young people in Out-of-Home Care through case management therapeutic planning and frontline coaching. Drive positive outcomes by embedding trauma-informed culturally safe practices and ensuring compliance with standards and legislation.

The role operates across two primary functions:

  • 18 hours per week supporting frontline staff in the homes to implement case and care strategies.
  • 20 hours per week completing case work duties including planning documentation and stakeholder engagement.

What Youll Be Doing:
Lead and review case plans to meet therapeutic and cultural goals
Coach and support staff to embed strategies in daily care
Engage with families carers and agencies to align goals
Maintain documentation and ensure compliance with standards
Promote safe nurturing and culturally respectful environments

What Youll Bring:
Diploma (or working towards) in community youth or social work
Proven case management and stakeholder engagement skills
Knowledge of trauma-informed and therapeutic care practices
Strong documentation and compliance capability
Current drivers licence and probity clearances
