Primary Service Areas: Livingston County

Schedule: Monday - Friday 8am - 5pm 40 hours a week

No weekends or holidays!

Position Purpose:

Our HHA/CNA provides paraprofessional services to the organizations home care clients as developed by the primary nurse and as prescribed by their physician and in compliance with the states Nurse Practice Act and any other regulations defining home health aide / hospice aide services.

Position Highlights:

  • Follows care plan developed by RN.

  • Reports client changes in condition to RN as described on care plan.

  • Assists clients with household tasks including planning and preparing meals feeding light housekeeping changing linens making bed and laundering clothes and bedding.

  • Measures and records clients oral rectal and axillary temperatures pulse respiration and blood pressure as ordered.

  • Performs range of motion and other simple procedures as an extension of therapy services as assigned.

  • Assists clients with use of equipment (side rails foot stools wheelchairs etc.) to meet safety needs.

  • Performs or assists clients to perform activities in accordance with the personal care services plan such as changing ostomy appliances or external catheter care.

  • Participates in multi-disciplinary case conferences as appropriate.

  • Accurately documents services provided in accordance with agency policy on day services are rendered.

  • Submits documentation in a timely manner.

  • Complies with agency policies and procedures.

  • Works with scheduling to assure accurate and up to date schedule.

Minimum Qualifications:

  • High School diploma or GED required.

  • Completion of federally compliant training program as required by law/regulation and successful completion of a competency evaluation program.

  • Licensed CNA within the State of Michigan

  • Must have current Drivers license and reliable transportation to and from work site.

  • Documentation of one years employment as an aide in an institution home care or hospice setting.

  • Meets agency hiring criteria as defined in agency minimum standards (e.g. provision of professional references.)

  • Must accept responsibility for maintaining skills learning and adhering to agencies policies and procedures on an on-going basis.

  • Ability to consistently demonstrate the Organizational Core Competencies and Organizational Code of Ethics and adhere to the Compliance Program.
