What Youll Do:
The Transition Specialist Outreach (TSO) will be responsible for building relationships awareness and support to disconnected youth on the streets as well as through contact with other service providers schools and community centers. Outreach is done both in a vehicle and on foot in targeted areas throughout The Relatives service area. The TSO helps young people by reducing the risks to being victimized improving their health and connecting them with needed services and support.

Education and Experience
- Bachelors degree in a human services field from an accredited college or university and one year directly related experience or bachelors degree from an accredited college or university and two years directly related experience.
- Proficiency in MS Office applications.
- Valid Drivers License
