Are you a compassionate and skilled Nurse Practitioner with a passion for providing exceptional wound care We are seeking a dedicated Mobile Woundcare Nurse Practitioner to join our team in beautiful Thousand Oaks. This is an exciting opportunity to make a significant impact on patients lives by delivering high-quality patient-centered care right where they need it the most.
Key Responsibilities:
- Provide comprehensive wound care to patients in various settings including homes and clinics ensuring optimal healing and comfort.
- Assess evaluate and manage patients wound care needs developing personalized treatment plans.
- Collaborate with a multidisciplinary team to coordinate patient care and ensure the best outcomes.
- Educate patients and their families on wound care management and prevention strategies.
- Maintain accurate and timely documentation of patient care and progress.
Qualifications:
- Must have a Woundcare certification
- Current and unrestricted Nurse Practitioner license in the state of California.
- Certification in wound care or relevant experience is preferred.
- Excellent clinical skills and the ability to work independently.
- Strong communication and interpersonal skills.
- Valid drivers license and reliable transportation for travel within the Thousand Oaks area.
