Job Summary

Primary Care Physician opportunity in Rochester NY

Wonderful full-time primary care role providing patient care in a post-acute care setting in a skilled nursing facility (SNF) located in the Rochester New York area. You will round 4-5 days a week with the flexibility to choose your rounding schedule.

Position highlights:

Primary care rounding in skilled nursing facilities
Manage chronic disease diagnose illnesses and medical conditions and implement plan of care
Collaborate with clinicians facility leaders and others regarding care and services
Provide oversight guidance and mentorship to our nurse practitioners
Conduct clinical case reviews as needed and share tools information and guidelines as they relate to quality of care

Qualifications:

Minimum of 1 year primary care experience
Post-acute care experience a plus
Active and unrestricted New York license or application in process
Active and unrestricted DEA/prescriptive authority or application in process
