  • Salary:34434 per annum (pro rata for part time)
  • Hours:37 hours per week flexible working options available

Your role and responsibilities

We offer support and a chance to work in an interesting environment as part of a busy integrated team with other health professionals.

You will play a key part in promoting personalisation independence wellbeing and choice by providing a social care service for people. You will be expected to exercise a high level of autonomous professional judgement and risk evaluation taking responsibility for the safety and well-being of people accessing adult and community services within a health environment.

The role will include completing asset-based assessments/reviews short term retirement plans care and support plans and safeguarding plans in line with Supporting Lives Connecting Communities and using a Signs of Safety and Solution Focused approach.

You will need

  • A strong working knowledge of relevant legislation including The Care Act 2014 and The Mental Capacity Act 2005 and how these apply in practice.
  • A solid understanding of Adult Social Care Services including safeguarding procedures and the assessment support planning and care management process.
  • Proven experience in applying a person-centred enabling approach to support individuals in maximising their independence and wellbeing.
  • Confidence in managing risk and safeguarding concerns including the development of safety plans to reduce the risk of ongoing abuse.
  • Experience of working collaboratively within integrated health and social care teams including Social Workers Occupational Therapists Nurses and other professionals.
