Your role and responsibilities
We offer support and a chance to work in an interesting environment as part of a busy integrated team with other health professionals.
You will play a key part in promoting personalisation independence wellbeing and choice by providing a social care service for people. You will be expected to exercise a high level of autonomous professional judgement and risk evaluation taking responsibility for the safety and well-being of people accessing adult and community services within a health environment.
The role will include completing asset-based assessments/reviews short term retirement plans care and support plans and safeguarding plans in line with Supporting Lives Connecting Communities and using a Signs of Safety and Solution Focused approach.
