Beth Israel Deaconess Medical Center is growing our transplant program and seeking a Clinical Pharmacy Specialist position with advanced clinical management and leadership training and experiences to innovate and advance a clinical pharmacy practice model for abdominal transplant services. The Clinical Pharmacy Specialist serves as a leader in clinical pharmacy practice in solid organ transplant. This position is the Clinical Pharmacy Specialist in solid organ transplant and will rotate between both inpatient care teams and ambulatory clinic direct patient care as a team of three pharmacists. Specialists provide comprehensive clinical and distributive medication management services and will utilize their advanced clinical skills in the provision of evidenced-based safe and cost-effective drug therapy to achieve optimal medication therapy outcomes. They serve as a drug information and educational resource for the pharmacy staff patients residents/students medical staff nursing staff and other healthcare professionals. The Clinical Pharmacy Specialist will participate in the development of clinical guidelines/protocols new programs/services and facilitate professional development activities in their area of expertise.

The Institutes kidney liver and pancreas programs consistently exceed volume and quality benchmarks contributing to system-wide growth and national recognition.

Job Description:

1. Practices in accordance with the state and federal laws rules and regulations including but not limited to Massachusetts Board of Registration in Pharmacy Department of Public Health the Food and Drug Administration and the Drug Enforcement Administration. Maintains certification and credentials in accordance with the Board of Registration and adheres to institutional policies and procedures.

2. Reviews and verifies medication orders with consideration including but not limited to the following patient specific parameters: diagnosis weight/body surface area age allergies organ function laboratory values medication duplication drug interactions. Clarifies orders deemed to be inappropriate incomplete or otherwise unclear with the prescriber in a timely manner.

3. Documents and communicates pertinent information and discussion with the healthcare team and pharmacy staff to ensure continuity of care.

4. Provides drug information to physicians nurses and other health care professionals.

5. Checks and verifies sterile and non-sterile medications.

6. Supervises the activities of students technicians and residents within the pharmacist area of practice.

7. Promotes a culture of patient safety by submitting medication related events and near misses.

8. Completes daily pharmacy surveillance tool monitoring including follow-up and documentation as well as other daily patient care initiatives and relevant reports.

9. Works collaboratively with all pharmacists pharmacy technicians and pharmacy personnel. Upholds a culture of respect and teamwork across the Pharmacy Department.

10. Precepts pharmacy students resident rotations and resident-led quality improvement projects. May serve as Residency Program Coordinator or Residency Program Director

11. Regularly participates in department initiatives including but not limited to supporting formulary stewardship presenting new medication requests at Pharmacy & Therapeutics Committee creating and maintaining policies and procedures conducting assessments of workflows and/or developing departmental tools and resources.

12. Supports a specific core area of practice through interdisciplinary education ongoing specialty training and as a resource for Pharmacy staff.

13. Updates clinical guidelines and presents hospital wide educational initiatives.

14. Serves on hospital interdisciplinary committees or workgroups to improve process and quality outcomes.

15. Supports IRB-approved research activities medication use evaluations or cost-savings initiatives.

16. Other duties as assigned

Required Qualifications:

1. PGY 1 1-3 years OR PYG2 OR 3-5 years of experience in clinical specialty area

1. BPS

Pay Range:

$61.53 - $78.04

The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors that may include seniority education training relevant experience relevant certifications geographyof work location job responsibilities or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials call pay premium pay overtime pay and other additional pay practices as applicable to the position and in accordance with the law.
