Principales Fonctions et Responsabilites:
- Servir de principale personne-ressource pour les patients
- Communiquer avec les patients pour recueillir de linformation et les renseigner sur les services offerts par le programme
- Servir dexpert en remboursement dans le territoire qui lui est confié
- Renseigner les patients sur les services du programme y compris sans sy limiter une formation sur lauto-injection les options en matière de pharmacie et le soutien au remboursement
- Assurer la coordination des services aux patients en temps opportun pour offrir un accès rapide et continu au traitement
- Travailler en collaboration avec les autres membres de léquipe dans divers rôles pour coordonner le cheminement du patient et assurer lintégration du patient
- Gérer tous les aspects de la couverture pour maximiser et maintenir la couverture dassurance du médicament dordonnanceInteragir régulièrement avec les compagnies dassurance les pharmacies et dautres professionnels de la santé pour offrir un soutien optimal en matière de remboursement
- Coordination des ordonnances (initiales et renouvellements)
- Signaler les effets indésirablesAutres tâches connexes qui lui sont confiées
12 Month Contract
Primary Duties and Responsibilities:
- Key point of contact for patients
- Contact patients to gather information and educate patients on program services
- Be a reimbursement expert in his/her assigned caseload territoryEducates the patient on program services including but not limited to injection training pharmacy options and reimbursement support
- Ensure coordination of patient services in a timely manner resulting in a rapid and continuous access to therapyWorks collaboratively with other team members in various roles to coordinate the patient journey and ensure patient onboarding
- Manage all aspects of coverage to maximize and maintain prescription drug coverage
- Regularly interacts with insurance companies pharmacies and other healthcare professionals to provide optimal reimbursement support
- Coordination of prescriptions (initial and renewal)Adverse Event reporting
- Ad hoc duties as assigned

Experience et Niveau dEducation Requis:
Exigences:
- Diplôme détudes postsecondaires ou grade dassocié ou une combinaison équivalente détudes et dexpérience
- Bilinguisme (anglais/français) est un atout (obligatoire pour les postes au Québec)
