We are currently seeking an organized detail-oriented and empatheticIntake Coordinator to join our Home Health Team to be the first point of contact for patients families and referral sources.

As an Intake Coordinator with our Home Health Team heres a big-picture view of what youll do:
- Plays a crucial role in the admissions process by handling new referrals
- Communicate & build relationships with physicians patients and families to gather necessary intake information
- Verifying patient information & insurance
- Coordinate with clinical and administrative teams
- Ensuring all documentation is accurate and complete for timely start of care & resumption of care
- Maintain HIPAA compliance and patient confidentiality at all times
A few must-haves for the Intake Coordinator role:
- High School Diploma or equivalent required; associate or bachelors degree preferred
- Excellent communication organizational and multitasking skills
- Ability to work in fast-paced environment and collaborate with a multidisciplinary team
- Previous experience in home health medical intake scheduling or healthcare administration
- Knowledge of Medicare Medicaid and Private Insurance guidelines
- Proficiency with EMR/EHR systems and Microsoft Office Suite
