Hospice Advanced Practice Provider (APP)
Goshen Home Care and Hospice is hiring a Board-Certified Advanced Practice Provider to join our Hospice team. This is a part-time position. Were looking for a passionate individual to work in a team-centered collaborative environment. Hospice provides end of life care to patients in the hospital home or collaboration with the hospice medical director this 0 .5 FTE role is responsible for the direction and coordination of Hospice medical care provided by the hospice care team.

Candidate Requirements:
- Must be Board Certified in Indiana.
- Hospice experience preferred but not required.
- Exhibit strong clinical skills and the ability to work independently within the scope of practice.
- Communicate well.
- After hours call coverage to consult with hospice and home care nurses regarding patient care and symptom management as scheduled.
- Knowledge of regulatory and legal landscape of Hospice practices as it relates to operations.
