Job Summary
Advanced Practice Provider
Full-Time Weekdays Only
Join a mission-driven community-based elder care program where you can make a meaningful impact every day. Were seeking a compassionate Nurse Practitioner or Physician Assistant to deliver high-quality primary care to older adults in a collaborative supportive team-based setting.
What Youll Do:
- Provide comprehensive routine and episodic assessments
- Manage acute and chronic medical conditions
- Conduct home and facility visits as needed
- Educate participants and ensure timely accurate documentation
- Participate in a limited shared on-call rotation
What We are Looking For:
- Licensed NP or PA in Massachusetts
- Experience working with geriatric populations preferred
- Strong clinical judgment communication and teamwork skills
