Job Summary

Overview

Join our client as an Admissions/SOC RN where you will play a vital role in providing compassionate care to patients in their homes. This position offers a structured schedule with opportunities for additional earnings through on-call duties if desired.

Key Responsibilities

Provide admissions and start-of-care nursing services to patients in a home setting
Coordinate care with other members of the healthcare team to ensure continuity and quality of care
Ensure compliance with all applicable healthcare regulations and documentation standards

Schedule and Shift Details

M-F; 10a-6p (Start and end times vary from day to day expected to be available from 10am 6pm daily.)

Travel

Territory includes multiple counties in the surrounding areas.

Qualifications
Required

Hospice (1 years)
1 years of experience in ANY of the following:
- Home Health Care
- Epic Home Health
- Home Health
Management (1 years)
High School/GED
Valid Registered Nurse (RN) in Illinois (IL)
ALL of the following valid licenses/certifications:
- American Heart Association Basic Life Support (AHA BLS)
- American Heart Association Advanced Cardiac Life Support (AHA ACLS)
