School Based Service Coordinator

Location: Pittsburgh PA

Job Overview:

The Service Coordinator will provide advocacy assessment outreach and linkage to resources and services for individuals with behavioral health needs those involved in the juvenile justice system or youth at risk of educational setbacks. This role is ideal for a self-starter who excels in a collaborative environment and demonstrates initiative empathy and strong communication skills. Service Coordinators will actively manage caseloads coordinate with community resources and empower clients on their path to recovery.

Program Specific Description:

HSAO School-Based Service Coordinators provide service coordination to students in designated school buildings throughout Allegheny County. School Based Service Coordinators are responsible for interfacing with school personnel to identify students in need of intensive behavioral health support for the purpose of removing barriers that interfere with both educational and developmental success.

Key Responsibilities:

Client Support and Case Management

  • Serve as the Single Point of Accountability (SPA) for clients and families acting as the primary contact and advocate for services.
  • Build and maintain enduring relationships with clients through persistent outreach and engagement.
  • Develop comprehensive service plans tailored to individual strengths and needs regularly updating and adjusting as necessary.
  • Coordinate care across various providers including behavioral health drug and alcohol services educational supports and community resources.
  • Maintain monthly face-to-face contact with clients ensuring services are provided in line with program standards.

Collaboration and Advocacy

  • Work closely with families probation officers educational staff and community providers to address non-academic barriers to learning and promote recovery.
  • Facilitate interdisciplinary team meetings and court service plans providing follow-up and ensuring continuity of care.
  • Act as an advocate for clients helping remove systemic barriers resolving issues and fostering full inclusion within their community.

Documentation and Compliance

  • Complete assessments service plans and case notes within specified timeframes ensuring all documentation complies with state and agency regulations.
  • Maintain accurate records of all interactions including family meetings service linkages and follow-up activities in electronic health records.
  • Comply with HIPAA confidentiality policies and participate in regular audits and program evaluations.

Professional Development

  • Participate in ongoing training and development opportunities staying updated on best practices and trends in mental health juvenile justice and service coordination.
  • Attend school-based meetings and offer educational advocacy as needed ensuring clients have the support they need to thrive.

Required Skills and Qualifications:

  • Bachelors degree in Social Work Psychology Education Criminal Justice or a related field (Masters degree preferred).
  • 2 years of experience in a behavioral health or community service setting (or equivalent experience with youth and families involved in the juvenile justice system).
  • Strong organizational skills with the ability to manage multiple priorities and meet deadlines.
  • Excellent communication and interpersonal skills with the ability to work independently and as part of a multidisciplinary team.
  • Demonstrated cultural competence and the ability to work with diverse populations.

Additional Requirements:

  • Valid Pennsylvania drivers license with access to a reliable vehicle.
  • Current auto insurance that meets minimum collision coverage requirements.
  • Act 33 Act 34 and FBI Clearances (including Institutional Clearances if working with incarcerated individuals).
  • Ability to complete required onboarding training within 30 days of hire.

Work Conditions and Benefits:

  • Full-time position (40 hours per week) with occasional evening or weekend hours to meet client needs.
  • $24.52 per hour ($51000 yearly)
  • PTO and Holidays: 18 days of paid time off plus 10 holidays annually.
