Center for Human Services (CHS) is seeking reliable and adaptable people to join our Operations team as Case Manager - CSA Bilingual English/Spanis.This is a full-time position working Monday through Friday 8am - 5pm.

POSITION SUMMARY: Assist parents in the Intensive Family Maintenance (FM) and Intensive Family Reunification (FR) program in collaboration with Community Services Agency (CSA).
DUTIES:
- Advocates for and assists adult clients in accessing and receiving mental health treatment medical care housing appropriate educational services and clothing as indicated.
- Develops individual service plans for each client.
- Provides guidance to clients in reaching their goals and monitors progress toward goals.
- Meets regularly with clients in the field at homes schools established Mental health Department sites established CHS sites and other appropriate sites.
- Consults with or meets regularly with the clients social worker care provider or others with the clients consent.
- Coaches clients in developing skills including social conflict resolution organization self-reliance money management and personal hygiene.
- Assists in establishing clients eligibility for Medi-Cal or other benefits that may be available to client. Advocates for continuation of benefits as needed.
- Transports clients in Agency owned vehicles as needed and approved.
- Interacts productively within a multi-disciplinary team.
- Assists with contact liaison and development of close working relationships with agencies schools businesses etc.
- Coordinate and develop written reports as assigned and required.
- Other duties as assigned.
Qualifications
QUALIFICATIONS:
- BA/ BS in a related field with 2 years experience in crisis intervention group family and individual counseling or working in a psychiatric inpatient setting. -or-Fifteen (15) units of college credit in a related field and four (4) years of progressively responsible experience in crisis intervention group family and individual counseling or working in a psychiatric inpatient setting -or-
Thirty (30) units of college credit in a related field and three (3) years of progressivelyresponsible experience in crisis intervention group family and individual counseling or working in a psychiatric inpatient setting.
- Bilingual - English/Spanish required. Ability to speak read and write in Spanish fluently
- Must adhere to Center for Human Services Human Resources requirements including:
- Pass agency paid criminal justice screening including fingerprints.
- Pass agency paid TB (Tuberculosis) screening.
- Pass agency paid health screening and/or drug testing. (if required).
- Valid California drivers license and proof of auto insurance coverage required.
- Pass agency paid MVR clearance with no more than 2 points - if you are under the age of 25 our insurance company will not allow any points
- Must adhere to Stanislaus County Community Services Agency Human Resources requirements including:
- Community Services Information Technology Security Policy.
- Federal Bureau of Investigation (FBI) and Department of Justice (DOJ) fingerprint clearance processed through County.
- Pass a criminal background clearance.
- California Law Enforcement Telecommunications System (CLETS) requirements.
- Pass all requirements to gain access and work in program specific buildings.

- Will promote and support a culturally and linguistically diverse workforce and be responsive to the population within our service area.
- Maintain confidentiality and confidential information in accordance with legal standards and/or agency regulations.
- Participate in assigned scheduled agency meetings in-service trainings conferences and other trainings as determined by the supervisor. This includes serving as an agency representative at assigned community meetings.
- Observance of assigned working hours and program appointments by demonstrating promptness and thorough preparation.
- Performance of assigned duties with a positive attitude and in the spirit of teamwork collaboration and cooperation.
- Communicate effectively both orally and in writing.
- Perform job duties in a safe manner to ensure a safe working environment for oneself and others.
- Participates in and/or supports agency fund development activities and events.
- Preparation of assigned reports work records statistical data job performance evaluations work plans a timely manner.
