Description

Center for Human Services (CHS) is seeking reliable and adaptable people to join our Operations team as Case Manager - CSA Bilingual English/Spanis.This is a full-time position working Monday through Friday 8am - 5pm.

POSITION SUMMARY: Assist parents in the Intensive Family Maintenance (FM) and Intensive Family Reunification (FR) program in collaboration with Community Services Agency (CSA).


DUTIES:

  1. Advocates for and assists adult clients in accessing and receiving mental health treatment medical care housing appropriate educational services and clothing as indicated.
  2. Develops individual service plans for each client.
  3. Provides guidance to clients in reaching their goals and monitors progress toward goals.
  4. Meets regularly with clients in the field at homes schools established Mental health Department sites established CHS sites and other appropriate sites.
  5. Consults with or meets regularly with the clients social worker care provider or others with the clients consent.
  6. Coaches clients in developing skills including social conflict resolution organization self-reliance money management and personal hygiene.
  7. Assists in establishing clients eligibility for Medi-Cal or other benefits that may be available to client. Advocates for continuation of benefits as needed.
  8. Transports clients in Agency owned vehicles as needed and approved.
  9. Interacts productively within a multi-disciplinary team.
  10. Assists with contact liaison and development of close working relationships with agencies schools businesses etc.
  11. Coordinate and develop written reports as assigned and required.
  12. Other duties as assigned.

QUALIFICATIONS:

  1. BA/ BS in a related field with 2 years experience in crisis intervention group family and individual counseling or working in a psychiatric inpatient setting. -or-Fifteen (15) units of college credit in a related field and four (4) years of progressively responsible experience in crisis intervention group family and individual counseling or working in a psychiatric inpatient setting -or-

Thirty (30) units of college credit in a related field and three (3) years of progressivelyresponsible experience in crisis intervention group family and individual counseling or working in a psychiatric inpatient setting.

  1. Bilingual - English/Spanish required. Ability to speak read and write in Spanish fluently
  2. Must adhere to Center for Human Services Human Resources requirements including:
    1. Pass agency paid criminal justice screening including fingerprints.
    2. Pass agency paid TB (Tuberculosis) screening.
    3. Pass agency paid health screening and/or drug testing. (if required).
    4. Valid California drivers license and proof of auto insurance coverage required.
    5. Pass agency paid MVR clearance with no more than 2 points - if you are under the age of 25 our insurance company will not allow any points
  3. Must adhere to Stanislaus County Community Services Agency Human Resources requirements including:
    1. Community Services Information Technology Security Policy.
    2. Federal Bureau of Investigation (FBI) and Department of Justice (DOJ) fingerprint clearance processed through County.
    3. Pass a criminal background clearance.
    4. California Law Enforcement Telecommunications System (CLETS) requirements.
    5. Pass all requirements to gain access and work in program specific buildings.

REQUIREMENTS:

  1. Represent the agency in a professional and competent manner.
  2. Advocate for the best interests of the agency and clients we serve.
  3. Establish and maintain effective working relationships with the general public co-workers clients supervisors and members of diverse cultural and linguistic backgrounds regardless of race color creed religion gender sexual orientation gender identity or expression national origin age ancestry political affiliation citizenship disability medical conditions marital status amnesty and military or veteran status.
  4. Will promote and support a culturally and linguistically diverse workforce and be responsive to the population within our service area.
  5. Maintain confidentiality and confidential information in accordance with legal standards and/or agency regulations.
  6. Participate in assigned scheduled agency meetings in-service trainings conferences and other trainings as determined by the supervisor. This includes serving as an agency representative at assigned community meetings.
  7. Observance of assigned working hours and program appointments by demonstrating promptness and thorough preparation.
  8. Performance of assigned duties with a positive attitude and in the spirit of teamwork collaboration and cooperation.
  9. Communicate effectively both orally and in writing.
  10. Perform job duties in a safe manner to ensure a safe working environment for oneself and others.
  11. Participates in and/or supports agency fund development activities and events.
  12. Preparation of assigned reports work records statistical data job performance evaluations work plans a timely manner.
