What Youll Do in This Role:


The Clinical Pharmacist at Huron Valley PACE plays a key role in optimizing medication therapy for older adults through participant-centered clinical services. This role supports the Interdisciplinary Team by identifying and resolving drug-related issues ensures compliance with Part D regulations and promotes cost-effective medication use. Youll also collaborate with hospitals pharmacies and leadership to close care gaps enhance outcomes and support continuous quality improvement.

Primary Responsibilities:

Participant Care:

  • Conduct comprehensive assessments to support individualized care planning treatment strategies and discharge coordination with the Interdisciplinary Team (IDT).
  • Review and evaluate drug information to identify potential issues and recommend evidence-based solutions.
  • Monitor participants responses to drug therapy including chronic conditions such as anticoagulation and diabetes under collaborative practice agreements.
  • Communicate changes in participant status to the IDT and update care plans accordingly.
  • Provide medication counseling to participants caregivers and families regarding proper use side effects and adverse reactions.
  • Document care activities and educational interactions accurately and on time.
  • Educate healthcare professionals pharmacists and students on optimal drug therapy practices including serving as a preceptor.
  • Participate in experiential and didactic training for pharmacy students residents and support team members.
  • Promote a culture of service excellence and ensure high levels of participant satisfaction.

Medication Review & Utilization Management:

  • Lead comprehensive medication reviews and support initiatives to optimize medication use.
  • Develop and implement policies to ensure compliance with Medicare Part D regulations.
  • Implement safety and quality improvement processes to enhance medication use and reduce costs.
  • Monitor prescribing patterns across multiple sites and provide education as needed.
  • Review current clinical literature to identify opportunities for improving drug therapy and participant outcomes.
  • Conduct audits and report findings to leadership.
  • Assist with compiling and analyzing data for Medicare Part D bids.

Staff Oversight:

  • Supervise and guide support staff including pharmacy technicians ensuring effective workflow and compliance.
  • Recruit onboard train mentor and address performance issues to maintain high standards of clinical service delivery.

What Youll Need:

  • Registered Pharmacist in the State of Michigan with a valid Controlled Substance license.
  • Doctor of Pharmacy (PharmD) degree or equivalent experience with advanced clinical training and knowledge in participant care and applied therapeutics including therapeutic drug monitoring.
  • PGY1 Pharmacy Practice Residency focused on ambulatory care or geriatrics preferred.
  • Board Certification in Geriatric or Ambulatory Pharmacy preferred.
  • Demonstrated knowledge of managed care progressive pharmacy practice and ability to meet standardized competencies to work independently under collaborative practice agreements.
  • Leadership experience with proven skills in guiding and mentoring staff desired.
  • Reliable transportation and valid Michigan drivers license.
  • Ability to lift and move objects weighing up to 35 pounds without assistance.
  • CPR certification desired or must be obtained within three months of employment.
