PRIMARY FUNCTION
The pharmacist works as part of an interdisciplinary team to dispense and compound medication pursuant to a valid prescription written by a heath care provider. The pharmacist provides education to patients about their medication its use and reviews DUR. The pharmacist is also responsible for the daily pharmacy operations.

PRINCIPLE DUTIES
  • Reviews prescriptions issued by authorized prescriber to assure accuracy proper filling and a prospective drug utilization review for both new and refilled prescriptions.
  • Counsels patients on medication indications proper use side effects interactions and storage in compliance with state and federal law.
  • Actively participates in patient care by offering therapeutic options or alternatives to physician.
  • Serves as a medication information resource to providers.
  • Works with financial counselors/ enabling services to find alternative payment routes for patients unable to afford necessary medication and treatments.
  • Analyzes records to indicate prescribing trends and excessive usage of medications.
  • Monitors drug use according to medication formulary and periodically reviews formulary for necessary changes.
  • Provides drug therapeutic monitoring to patients on certain medications in collaboration with the physicians.
  • Reconciles third party payment records and investigates any discrepancies.
  • Monitors inventory and verifies the correct stock is being used.
  • Develops White House Clinics pharmacologic knowledge by participating in clinic education training pharmacy staff students and other health care professionals.
  • Participates in medication therapy management.
  • Engages in the Coumadin Clinic by monitoring adjusting and coordinating all aspects of the patients anticoagulation therapy and follow up appointments.
  • Manages daily pharmacy operations and supervises pharmacy staff by directing technicians work flow and performing other administrative duties.
  • Participates in all safety programs which may include assignment to an emergency response team.
  • Performs other duties or services deemed necessary.
  • Acknowledges that White House Clinics strives to be a patient-centered medical home and as such prescribes to team-based delivery of primary care services. Recognizes that the completion of the above listed duties illustrates the employees role in aforementioned team-based care and accepts responsibility for being an active member of the team including identification of quality improvement opportunities.

  • Minimum Education:Bachelor of Science in Pharmacy or Pharm. D. degree
  • Minimum Work Experience:N/A
  • Required License:Active Kentucky Pharmacy License
  • Qualifications
    • Successful candidates must demonstrate skill in exercising a high degree of initiative judgment and discretion. Must be able to analyze situations accurately and take effective action while establishing and maintaining effective working relationships. Successful candidates must also be adept at planning and organizing in order to prioritize activities and meet deadlines. Finally candidates must be able to communicate effectively both orally and in writing to a varied audience.

    • Maintains compliance with organizations policies as well as established practices protocols and procedures of the position department and applicable professional standards. Complies with organizational and regulatory policies for handling confidential patient information. Adheres to professional standards clinic policies and procedures (OSHA HIPAA Standing Orders etc.) federal state and local requirements Health Resources and Services Administration (HRSA) U.S. Department of Health and Human Services (HHS) Office of the Inspector General (OIG) etc. Cooperates fully with all aspects of Corporate Compliance Plan and Standards of Conduct. Participates in all safety programs which may include assignment to an emergency response team.

SUPERVISION EXERCISED:None.

TYPICAL PHYSICAL DEMANDS
May require prolonged sitting and/or standing some bending stooping stretching and lifting (generally not in excess of 50 pounds at a time). Requires hand-eye coordination and manual dexterity sufficient to operate a keyboard photocopier telephone calculator and other office equipment. Requires normal range of hearing and eyesight to record prepare and communicate appropriate reports.

TYPICAL WORKING CONDITIONS
Work is performed in a pharmacy within the clinics office environment. It requires frequent contact with other pharmacy staff as well as other health care professionals and the public. This position is exempt from wage and hour laws and may require work weeks in excess of forty hours and/or an irregular schedule occasionally. Work may be stressful at times.

EVALUATION
The pharmacist will be evaluated by the Pharmacy Director with input from other employees of White House Clinics as deemed necessary.
