Job Summary
Work Shift:
Day - 8 Hour (United States of America)8:00am - 4:30pm (40 Weekly Hours) / Weekends as NeededThe Social Worker is responsible for completing bio psychological assessments of patients and development of patient care plans for post discharge services. Additional discharge planning activities include but are not limited to referrals to home health DME dialysis short-term rehabilitation and nursing home placements. Also services as a resource for staff patients and families with regard to community agencies. Manages adoptions and assists with special programs and education.
Education & Training:
Masters Degree in Social Work.
Experience:
Two (2) years experience in hospital or community social work. Case Management experience.
Required Certification/Registration/Licensure:
None.
Knowledge Skills & Abilities:
Knowledge of medical and social work terminology. Utilizes a variety of standard office equipment to include computers printer fax machine CRT system etc.
