About the Role
An exciting opportunity currently exists for a physiotherapist to join the Health Independence Programs (HIP) team. The physiotherapist will deliver high quality person-centred goal-orientated and home and centre-basedcare within a supportive multidisciplinary team.
This position may be required to work at any of the HIP hub sites or programs. This will be at the discretion of the HIP manager and will take into consideration appropriate skills and training.

About the Department
This position is part of the Health Independence Programs (HIP). The intent of HIP is to deliver improved outcomes for clients and to support hospital demand management and flow through delivering integrated and coordinated care to clients across the hospital and community interface reducing duplication and fragmentation of services and ensuring client centered care planning.
St Vincents Hospital Melbourne (SVHM) HIP consists of integrated hub sites across several locations including in Fitzroy Kew North Richmond and Fairfield.
HIP at SVHM is delivered in partnership with North Richmond Community Health (NRCH).
This position is part of the HIP Community Rehabilitation team. Community rehabilitation is currently provided from our centers at Kew and Fairfield.
The key aim of Community Rehabilitation is to help people who are recovering from traumatic injury or illness are frail or chronically ill to regain optimal function and to allow people to maximize their independence.
The programs offer a supportive and dynamic team environment which focusses on patient-centered care.

Position Description
LOCAL WORK ENVIRONMENT
This position is part of the Health Independence Programs (HIP). fragmentation of services and ensuring client centred care HIP consists of integrated hub sites across several locations including in Fitzroy Kew NorthRichmond and Community Rehabilitation Services provide time-limited goalorientated care for people who have aphysical disability are frail chronically ill or recovering from traumatic injury or illness to regain and /ormaintain optimal function. Community Rehabilitation services comprise of Community RehabilitationCentres at Fairfield and Kew and Rehabilitation in the position may require working from any of the SVHM Health Independence Program (HIP) sites andmay be required to work within any of the HIP programs. This will be at the discretion of the HIP managerand will take into consideration appropriate skills and training. HIP at SVHM is delivered in partnership with North Richmond Community Health (NRCH).
POSITION PURPOSE
To work within a flexible interdisciplinary structure that ensures the delivery of high quality therapyservices to all clients of HIP Rehabilitation services making sure that the clients needs are met and theyreceive the right treatment in the right place at the right position will also be responsible for driving standards of care within the rehabilitation team includingleading and contributing to improvement projects within the are required to provide assessment and management of clients in a number of settings across
the continuum of care within HIP including clients residence community settings the HIP Centers orSpecialist Clinics.
POSITION DUTIES
Assess and manage all assigned clients as appropriate according to HIP and St. Vincents policiesand procedures and relevant rehabilitation protocols. There is a significant clinical component ofthis role.
Provide clinical leadership in maintaining an evidence and outcomes based service.
Review the existing Physiotherapy services within Community Rehabilitation and lead servicedevelopment and improvement opportunities
Supervision guidance and support to HIP Physiotherapists as required.
Achieve activity KPIs for relevant service
Participate in relevant in-service programs and meetings
Provide teaching to physiotherapy students
Maintain accurate statistics and submit by end of working week/ end of month (whicheveroccurs first)
Maintain the appropriate standard of medical record keeping and data collection as per and HIP policies
INCUMBENT OBLIGATIONS

8. SELECTION CRITERIA
8.1 ESSENTIAL REGISTRATION LICENSE OR QUALIFICATION REQUIREMENTS
Bachelor of Physiotherapy or equivalent
Current Registration with the Australian Health Practitioner Regulation Agency (AHPRA)
Eligibility for Australian Physiotherapy Association (APA) membership
Current Victorian drivers licence
8.2 OTHER ESSENTIAL REQUIREMENTS
Minimum of eight (8) years post-graduate experience in a relevant field of rehabilitation
Demonstrated highly developed knowledge and application of physiotherapy practice andprinciples in the area of rehabilitation
Demonstrated highly developed clinical management skills
Demonstrated commitment to service improvement and professional development
Highly developed leadership and interpersonal skills that incorporate excellent communication
