Case Worker: Out-of-Home Care

We are looking for a skilled and compassionate Case Worker to join our Rockhampton team and provide therapeutic trauma-informed support to children and young people living in Out-of-Home Care (OOHC).
About the Role
As a Case Worker you will play a pivotal role in supporting children and young people to achieve their goals and thrive. You will:
- Work alongside front-line staff (18 hrs/week) to implement therapeutic and case strategies in the home.
- Delivering case work services (20 hrs/week) including planning documentation and stakeholder engagement.
- Develop strength-based culturally safe case plans that promote stability wellbeing and long-term success.
- Coordinate services monitor progress and ensure compliance with OOHC requirements.
- Support young people in daily life skills transitions and community engagement.
- Collaborate with Therapeutic Specialists to ensure consistent care strategies.
- Lead life story work permanency planning and family connection activities.
- Provide coaching and leadership to care staff fostering a safe and nurturing home environment.
This is a dynamic role with a balance of direct engagement case planning and staff support.
About You
You are passionate about working with children and young people and bring experience in case management therapeutic support and stakeholder collaboration. Youll also have:
- A relevant qualification (Diploma or higher).
- Understanding of trauma-informed and culturally responsive practice.
- Strong communication documentation and relationship-building skills.
- A current unrestricted drivers license.
- Ability to meet probity and compliance requirements.
