Job Summary:
This temporary-to-permanent position requires a Licensed Social Worker (LICSW LSWAIC or LASW) to provide social work services to geriatric participants in an outpatient facility. The role involves direct patient care including individual and group counseling assessment care planning and accessing community resources. A moderate amount of home visits within a specified geographic area is required.
Location: Seattle Washington United States
Responsibilities:
- Provide individual and group counseling to geriatric participants.
- Conduct comprehensive social work assessments considering social emotional cognitive and ethnic factors.
- Develop and implement individualized integrated plans of care.
- Access and utilize community and financial resources for participants.
- Ensure continuity of care across the continuum.
- Conduct home visits as needed to assess participants needs and provide interventions.
- Collaborate with the interdisciplinary team.
- Maintain accurate and timely documentation.
- Travel to participant homes within a defined geographic area (Edmonds to Auburn).
Required Skills & Certifications:
- Masters degree in Social Work (MSW).
- Active WA LICSW WA LSWAIC or WA LASW license (temporary licensure acceptable).
- Minimum of two years of experience working with a geriatric population.
- Basic Life Support (BLS) certification.

Special Considerations:
- Temporary-to-permanent position (13-week contract with potential for permanent employment).
- Open to both local candidates and travelers including first-time travelers.
- Moderate amount of home visits within a specified geographic area (Edmonds to Auburn).
- Mileage reimbursement for travel between work locations (not for commute).
- All travel occurs during paid work hours (8 am - 5 pm).
- Significant autonomy in scheduling home visits.
- RTO policy to be confirmed with the hiring manager.
Scheduling:
- Three 8-hour days per week.
- Specific schedule details to be determined.
- No weekend or on-call requirements.
