Job Title: Social Worker - Complex Care
Section/Team: Complex Care Team
Location: London Borough of Islington
Pay Rate: 33.73 per hour
Job Description:
Are you passionate about supporting adults with complex needs
If so Liquid Personnel is recruiting a Social Worker to join the Complex Adult Social Work Team in the London Borough of Islington.
Join a dynamic team supporting individuals with multiple and high-level needs including mental health issues substance misuse homelessness cognitive impairments and safeguarding concerns. Youll be working in a fast-paced multi-agency environment making a real difference to those with the most complex circumstances.
Key Responsibilities:
- Complete strengths-based assessments and risk management plans
- Lead on safeguarding enquiries under Section 42
- Coordinate multi-disciplinary interventions
- Apply the Care Act Mental Capacity Act and Human Rights legislation
- Work closely with health housing and voluntary sector partners
- Maintain accurate records using insert system e.g. Liquidlogic/Mosaic
