Description

Humboldt Senior Resource Center is looking for a full-time Transportation Coordinator to support the transportation services for our Adult Day Health Services and Redwood Coast PACE program. The schedule for this position is Monday through Friday 9 am to 5:30 pm.

JOB SUMMARY: The Transportation Coordinator is responsible for assisting the Transportation Supervisor with day-to-day department operations administrative tasks and reporting. The Transportation Coordinator is also responsible for Redwood Coast PACE Medication delivery.

ESSENTIAL DUTIES AND RESONSIBILITIES:

  1. Answers screens and transfers phone calls and emails as appropriate providing support for transportation Manager Supervisors and Dispatchers.
  2. Coordinates daily delivery services including but not limited to meals durable medical equipment samples and medications.
  3. In conjunction with Transportation Dispatch coordinates and schedules outside transportation vendors by facilitating information and back-up service when needed. Maintains and updates database of approved outside transportation vendors.
  4. Ensures a smooth delivery of service by problem solving and coordination with drivers representatives of agency programs and participants. Collaborates and proactively communicates across departments regarding pertinent items.
  5. Attends Transportation team meetings and in-service trainings and implements safety and agency policies into practice.
  6. Creates monthly program reports on delivery types participants transported participant cancellations/no shows and undeliverables.
  7. Maintains confidential information in relation to transportation of participants.
  8. Follows agency policies for incident reports grievances and appeals.
  9. Completes documentation accurately and in a timely manner.
  10. Provides Transportation Supervisor with recommendation for transportation department improvement needs.
  11. Assists Lead Drivers with scheduling regulatory compliance record keeping documentation and filing as needed.
  12. Operates the agencys transportation vehicles to transport pick up and deliver Redwood Coast PACE Medications.
  13. Reports any special situations in the participants home to the Transportation Supervisor and other appropriate team members as soon as possible.
  14. Utilizes a pre-trip checklist to ensure vehicles operate safely and effectively.
  15. Utilizes routing software to access driving directions general transportation information and to maintain logs of all route trips made.
  16. Promptly reports all accidents incidents and/or injuries to the Transportation Supervisor and completes required reporting.
  17. Performs all tasks according to safety protocols and immediately notifies management of any unsafe working conditions.
  18. Participates in the process of teaching training and mentoring student interns and volunteers working within the program.
  19. Acts in a manner consistent with agency values.
  20. Other Duties as assigned.

Qualifications

EDUCATION &/or EXPERIENCE:

  1. High School Diploma or equivalent.
  2. One year experience working with elderly or frail individuals preferred.

LICENSES OR CERTIFICATES:

  1. Current California Class C Drivers License.
  2. Accident-free driving record with no moving violations within the last three (3) years preferred.
  3. Current CPR and First Aid certification within six (6) months of hire.

PRE-SCREENING REQUIREMENTS:


  1. Requires clearance of a DOJ and FBI criminal history background check.
  2. Pre-employment physical and PPD skin test (TB screening).
  3. COVID-19 and Influenza (flu) vaccines preferred.
  4. Motor Vehicle Report required.

  21. Ability to understand and incorporate detailed requirements and implement them into action.
  22. Ability to interpret and comply with various requirements regulations and contracts. Possess sound judgement to interpret and apply agency policies and procedures.

  24. Possess excellent recordkeeping skills and ability to maintain accurate files and records.

PHYSICAL REQUIREMENTS:

Physical abilities sufficient to move between different staff work areas communicate with co-workers and the public operate a computer produce reports talk on the telephone travel to other agency move between different work areas operate a motor vehicle for extended periods of time and enter/exit a motor vehicle repeatedly throughout the workday.

The Humboldt Senior Resource Center is a private non-profit multi-purpose senior center providing a continuum of social and health related services. The agency currently holds contracts with the California Department of Aging and Area One Agency on Aging.
