Job Summary
PLEASE NOTE:This is a limited-duration benefited full-time position currently funded through December 31 2026. This position may be extended beyond this date and/or may become regular status depending on funding and budget approval.
JOB SUMMARY
The Justice-Involved Health Program Coordinator supports the HRSA Quality Improvement Fund - Transitions in Care for Justice-Involved Populations (QIF-TJI)grant by coordinating the delivery of services for individuals transitioning from carceral settings into the community. This positions work could take place in a clinical setting in the community or in the carceral setting.This role works closely with the re-entry provider to ensure immediate access to primary care behavioral health and substance use disorder services. It includes community connections case management and same-day appointments when possible.
The coordinator will build partnerships with community-based organizations corrections agencies and internal Community Health Center teams to address barriers support continuity of care and track program outcomes.
This position coordinates grant reporting requirements supports program development and assists with the creation and refinement of processes to ensure the programs effectiveness and sustainability. Following the terms timelines and deliverables to HRSA grants requires care attention and collaboration with this federal program.
The Community Health Centers of Benton and Linn Counties (CHC) is a public-entity CHC providing care to everyone in the community. Services include primary care behavioral health substance use services and dental care.

Annual salary range of:
- $65566.63 - $94774.96
- Note: salary placement will be based on your level of directly related experience and education consistent with Oregons pay equity law.
- This position guarantees two salary raises within your first year. Increases are given at the end of the 6 month probationary period and at the 1 year anniversary as long as your performance is in good standing! Increases are given until the maximum of the salary range is reached.

MINIMUM QUALIFICATIONS (PC42)
The following minimum qualifications arerequiredfor this position:- Bachelors Degree from an accredited school in Public Health public administration communications or a related field ;AND
- 1 years of professional work experience in public health program management or related experience.
