Position: Physical Therapy Assistant
Supervisor:Director of Clinical Rehabilitation Services Inpatient Clinical Supervisor
Hours: Full-time 8am-4pm
Hourly Rate: $20-$25 per hour commensurate with experience.
Responsibilities/Duties:
Is able to identify patient/resident care related problems and progress and changes in status and brings them to the attention of the supervising physical therapist.
Goal oriented treatment plans are carried out as established by the supervising physical therapist.
Performs restorative/rehab programs after evaluation is completed by physical therapist.
Assists with maintenance physical therapy programs.
Demonstrates knowledge of physical therapy techniques modalities equipment and exercise programs.
Clearly instructs patient/resident and/or family in appropriate exercises and home programs as well as recommends appropriate adaptations equipment and community resources previously reviewed by supervising physical therapist.
Instructs patients/residents/families in the application care and precautions of orthosis/prosthetics.
Collaborates with patient/resident families and supervising physical therapist prior to patient/resident discharge in order to formulate an appropriate discharge or follow-up plan.
Organizes schedule of patient/resident treatments taking into account the work demands of other employees/departments.
Collaborates with physical therapists regularly to enhance patient/resident care on issues of progression variations/ideas for treatment patient/resident/family education discharge plans and any other concerns or issues which may arise.
If attends continuing educational programs and/or equivalent annually and provides an inservice to staff.
Participates in program development/revision within the facility. May formulate and design new programs or services to department.
May serve as a clinical instructor in conjunction with physical therapist to physical therapy assistant student. Affiliates following the facility clinical education manual and academic program manual.
Actively participates in own performance appraisal evaluation and sets self-goals to accomplish during the following year. Successfully completes annual competency checklist/test.
Demonstrates a willingness to provide open communication and guidance and direction to fellow employees volunteers students and visitors.
After consulting with supervising physical therapist attends patient/resident care conferences team conferences and rounds obtaining and reporting patient/resident present level of functioning future goals/plans in a clear and concise manner as well as change in condition or treatment of patient/resident.
Engages in timely creative and effective use of aides and volunteers. Seeks assistance from physical therapists as appropriate.
Completes documentation for insurance requests in a timely fashion.
After consulting with physical therapist provides objective accurate and timely documentation according to policy on assigned patients/ residents via progress notes update/revise care plans team conference reports FIM and MDS sheets charge/ attendance checklist discharge summaries and all other miscellaneous facility forms as necessary. Has supervising physical therapist co-sign appropriate documentation.
Leaves a written clear treatment plan (coverage notes) on all assigned patients/residents when
scheduled out of the department. May use phone to address above as appropriate.
If on specific long-term care rotation writes quarterly progress notes on maintenance residents and bi-monthly on restorative residents and obtains co-signature of physical therapist.
Demonstrates initiative to assist in evaluating and development of current documentation systems forms and charting mechanisms to facilitate efficient and effective documentation.
Efficiently recommends equipment and/or supplies when inventory demonstrates the need to reorder.
Participates in department QA programs including collecting of data analysis and documentation review.
Assists supervisor in developing new innovative ideas for QA studies and participates in discussions.
Performs duties according to the highest ethical and moral standards and in adherence with the facilitys corporate compliance plan and all applicable governing laws.
Performs all other duties as assigned by supervisor to ensure that the department functions and operates as smoothly and efficiently as possible.
Qualifications:
Graduate with an Associate of Science degree from a Physical Therapy Assistant Accredited program.
Basic entry-level knowledge gained through normal course of education and student clinicals.
Possess a New York State license and have a New York State Certification to practice as a physical therapist assistant.
CPR certification.
Possession of or eligible for a valid Physical Therapy Assistant license issued by the State of New York CPR certification. If setting for licensure examination there is a condition of passing the licensure exam within one year after employment and during the interim possess a New York State temporary permit.
Successfully completed the National Certification Exam forPTAis preferred.
