Responsibilities:
- Accept and treat patients within your designated geographic area
- Provide physical therapy treatments to patients in their homes as outlined in the licensed PTs plan of care
- Monitor and report changes in patient status to the supervising PT in a timely manner
- Communicate effectively with patients caregivers and interdisciplinary team members to ensure coordinated effective care
- Instruct patients family members caregivers and Home Health Aides in exercises safety awareness and use of assistive devices as directed by the supervising PT
- Assist the PT in updating care plans based on patient progress and response to treatment
- Document patient treatments and observations accurately and promptly in accordance with agency and regulatory standards
- Participate in case conferences and contribute to the care planning and discharge process in collaboration with the PT
- Adhere to all federal state and local regulations as well as agency policies and professional standards of practice
Qualifications :
- Physical Therapist Assistant Program Graduate
- Licensed Physical Therapist Assistant in the state of New York or eligible.
- Mandatory Certifications: First Aid AED & CPR Certifications.
- Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
- Ability to travel to patients homes within the assigned geographic territory
