Join Ontarios leading organ and tissue donation agency and help transform lives. As a Chart Review Specialist youll play a critical role in identifying donation opportunities and improving system-wide performance. Your work will directly support strategic planning and public reporting helping to drive donation improvement across the province.

This is a part time (0.40 FTE)- Flexible work hours

Here is what you will be doing:

Chart Selection & Review

  • Identify patient encounters for review based on donation eligibility criteria.

  • Review deceased patient charts to determine organ/tissue donation potential.

  • Abstract and document relevant clinical information using TGLN standards.

Hospital Coordination & Scheduling

  • Organize review schedules with hospital partners while respecting health information protocols.

  • Travel to hospitals across Ontario and learn up to 15 different electronic medical record systems.

  • Maintain multiple logins and confidentiality agreements for each site.

Data Entry & Quality Assurance

  • Accurately enter abstracted data into the TGLN database.

  • Cross-reference hospital mortality lists with TGLN reports to identify charts for review.

  • Ensure data quality through portal report monitoring and follow-up with stakeholders.

  • Project work as assigned

Stakeholder Engagement

  • Collaborate with Organ and Tissue Donation Coordinators (OTDCs) for clarification and accuracy.

  • Communicate with hospital Health Information Management Specialists (HIMS) for coding support.

  • Request and interpret reports from Public Reporting Specialists or IT as needed.

Privacy & Compliance

  • Transport and manage sensitive information securely.

  • Uphold privacy confidentiality and Occupational Health and Safety standards.

  • Ensure compliance with the Accessibility for Ontarians with Disabilities Act (AODA).

Here is what you will need to be successful:

Education and Experience

  • Bachelors degree or Diploma in Science Arts Humanities or Management.

  • Successful completion of a recognized Health Information Management Program.

  • Currently certified with the Canadian Health Information Management Association (CHIMA) and in good standing.

Knowledge and Skills

  • Minimum of two (2) years of experience in related role or a combination of equivalent experience and education.

  • Demonstrated attention to detail and data integrity.

  • Demonstrated ability to work independently.

  • Demonstrated ability to organize and prioritize work to meet quality and productivity standards and workload demands.

  • Proficient in the use of Microsoft Office Applications specifically Excel and Outlook.

  • A valid G class drivers license and access to a reliable vehicle is required.

  • Proven knowledge of various electronic health information systems required.

  • Demonstrates a strong ability to extract information required for research.
