A faith-based nonprofit organization in Louisville is seeking a compassionate Case Manager to provide individualized support to individuals experiencing homelessness. This vital role combines advocacy care coordination and faith-based service to address housing mental health addiction and other barriers helping clients work toward stability and long-term independence.
Client Assessment & Planning
- Conduct comprehensive assessments of client needs including housing health income and personal goals.
- Develop individualized service and housing plans in collaboration with clients.
- Monitor client progress and adjust plans to support long-term stability.
Advocacy & Resource Connection
- Connect clients with appropriate community resources such as shelters healthcare mental health services and recovery programs.
- Advocate on behalf of clients with housing programs government agencies and service providers.
- Assist clients in navigating housing applications and overcoming barriers to permanent housing.
Crisis Intervention & Support
- Provide crisis intervention and de-escalation using trauma-informed and harm-reduction practices.
- Support clients in developing life skills financial literacy and self-advocacy to foster independence.
- Educate and empower clients around goal setting and resilience.
Collaboration & Documentation
- Work with multidisciplinary teams including healthcare providers outreach workers and housing specialists.
- Maintain accurate case documentation service plans and progress notes.
- Contribute to program development best practices and service workflows.
Learning Center Development
- Help establish a Learning Center by assessing client needs and skill gaps.
- Coordinate with community partners to source or develop classes in areas such as employment readiness household management and life skills.
- Bachelors degree in Social Work Psychology Human Services or related field (preferred).
- 13 years of experience in case management social services homeless services or behavioral health.
- Experience working with vulnerable populations impacted by homelessness mental illness substance use and trauma.
- Knowledge of trauma-informed care harm reduction and strengths-based approaches.
- Familiarity with local housing systems public benefits and community resources.
