Shared Health leads the planning and coordinates the integration of patient-centered clinical and preventive health services across Manitoba. The organization also delivers some province-wide health services and supports centralized administrative and business functions for Manitoba health organizations.

Position Overview

The Program of Assertive Community Treatment (PACT) is an outreach oriented comprehensive community treatment rehabilitation and support service designed to meet the needs of people with severe and persistent mental illness. The service is provided to participants in their homes at work and in community settings.

Services include attention to both mental health needs and basic needs such as everyday activities like caring for oneself taking medications establishing social relationships finding and keeping places to live and work as well as managing ones finances. Services enable participants to regain stability take steps towards recovery and achieve personal goals.

The Service Coordinator Housing will provide support and assistance to PACT participants in obtaining maintaining and keep housing. The service coordinator will work with individuals from all the PACT teams who are homeless at risk for homelessness or eviction and require more intensive support to keep their housing. The service coordinator will spend 2 days a week at PACT Logan assisting the tenants of 400 Logan in maintaining their housing and preparing them to move to more independent community housing. They will divide the remainder of their time between the other teams to assist those individuals with housing needs. The housing service coordinator will also develop and work in partnership with other community partners who can assist with housing.


Main Function:
The Service Coordinator - Housing PACT is responsible for assessing planning development and coordination of treatment rehabilitation and support services provided to all PACT program participants (Logan Hargrave Leila and Winnipeg West) under the clinical supervision of the team leader The service coordinator will provide services to individuals using the principles of psychosocial rehabilitation and recovery to guide their practice and function as a clinical member of the inter-disciplinary team.
This service coordinator will focus on assisting individuals with housing and act as a resource to the team around housing. The service coordinator will be responsible for recording liaising with community partners such as EIA PGTO provide education to clients and family delivery of oral medications. The housing service coordinator will engage in community outreach to develop relationships with housing agencies property managers and landlords to assist PACT individuals to find get and keep housing. he housing service coordinator will assist the participant in applying and viewing for apartments. If the participant obtains housing they will coordinate and manage the moves and liaise with the landlord as needed.

They will ensure that staff are aware of the housing stock available and who they should refer for additional housing supports.

Under the general supervision of the Team Leader and psychiatrist the Service Coordinator is responsible for providing case management services and supports to participants involved with the PACT teams.

Experience

  • Minimum four years related community experience working with people with severe and enduring mental illness; combination of institution and community experience will be considered.
  • Experience in assisting clients obtaining housing and working knowledge current resources and legislation will be considered an asset.

Education (Degree/Diploma/Certificate)

  • BSW OT PT BSc PN RPN BN RN SLP RD or related health/human service degree required.
  • Specific discipline required will be based on the operational needs of the inter-disciplinary team.

Certification/Licensure/Registration

  • Responsible for maintaining and providing proof of registration with relevant regulatory body.
  • Vehicle and drivers license required.

Qualifications and Skills

  • Working knowledge of the principles of psychosocial rehabilitation and recovery.
  • Skills in assessing planning development and coordination of services.
  • Working knowledge and experience in working with people experiencing addictions an asset.
  • Effective networking and agency relation skills.
  • Excellent organizational communication and interpersonal skills.
  • Familiar with the Residential Tenancies Act.
