Job Description
We are seeking a skilled Physical Therapist to join a skilled nursing and long-term care facility in Longview WA on a travel contract. The Physical Therapist will evaluate residents design personalized treatment plans and provide therapy interventions to improve mobility strength balance and overall independence. The ideal candidate is compassionate adaptable and experienced in geriatric rehabilitation.
Key Responsibilities
- Conduct evaluations to assess residents mobility strength and functional needs.
- Develop and implement individualized treatment plans in collaboration with physicians and interdisciplinary teams.
- Provide therapy services including gait training strengthening balance activities and functional mobility exercises.
- Educate residents families and caregivers on therapy goals safety practices and the use of assistive devices.
- Document evaluations progress notes and therapy sessions accurately and in compliance with facility and regulatory requirements.
- Collaborate with occupational therapy speech therapy and nursing staff to ensure patient-centered coordinated care.
- Support residents in regaining independence and improving quality of life through evidence-based physical therapy practices.
Qualifications
- Doctorate or Masters degree in Physical Therapy from an accredited program.
- Current and valid Physical Therapist license in Washington (or compact license).
- Minimum of 1 year of recent PT experience preferably in LTC/SNF settings.
- Current certification in Basic Life Support (BLS).
- COVID-19 vaccine and recent tuberculosis test.
- Strong clinical assessment documentation and communication skills.
- Compassionate resident-centered approach with geriatric rehab experience preferred.
