Position Summary:
Full Time
Shift: 8:30am - 5:00pm(Monday Friday)
Compensation: $40 - $46/hour - Based on experience
Responsible for evaluation plan of care treatment re-evaluation discharge documentation and modification of all patient/resident programs. Administers physical therapy in compliance with Federal State local and Bethany Village standards policies and guidelines.
Job Duties:
- Administers physical therapy in compliance with Federal State local and Bethany Village standards policies and guidelines ensuring quality care and service to Bethany Village residents.
- Interacts with health professionals residents families and support groups to ensure the maximum rehabilitation while serving as a client advocate at all times.
- Evaluates residents by administering muscle neurological gait and functional tests.
- Establishes treatment goals based on the evaluation of the resident in conjunction with the residents needs.
- Develops treatment programs and provides ongoing assessments and modification as changes in residents condition occur.
- Prepares transmits and maintains timely and accurate records to include the initial evaluation progress notes discharge summary and attendance records.
- Complies with OSHA standards and ensures the safety of the resident in their care at all times.
- Adheres to infection control policies professional therapeutic services policies and all Bethany Village policies and procedures.
- Adhere to all policies and procedures of the organization and all federal and state laws regarding the privacy of protected health information.
- Adhere to all policies and procedures of the organization regarding the use of technology and technology-based information.
- Responsible for welcoming and on-boarding new employees.
- Performs other duties as directed.
Qualifications:
Minimum Qualifications: Bachelors degree from an accredited college in Physical Therapy (Masters degree preferred). Current Ohio Physical Therapy License.
Minimum Experience: At least two years physical therapy experience in long-term health care setting.
