To provide & coordinate medical services to ensure highest standards of patient care. Work as an integral part of the team by providing administrative support and acting as liaison between clinicians and Health Information Services.
Principal Accountabilities
(A) Strategic
Implementation of hospitals /medical services vision & philosophy.
Supplement/take charge in case the vertical is on leave/absent etc.
Strategic inputs in her/his vertical & should forward recommendation for continuous improvement in the vertical.
(B) Operational
Transcribe dictation by physicians and other healthcare providers in order to document patient care.
Translate highly technical medical reports from a doctors dictation into typewritten medical records.
Use word processing equipment for typing. Using earphones to listen to the dictation compose directly from the voice recording.
Transcribes word processes patient histories and physicals various reports and death summaries.
Edits dictation so that it is consistent and understandable and supply correct format punctuation capitalization and spelling.
Maintain physician and correspondence files and perform clerical duties such as answering telephones record keeping and filing of reports.
Applies knowledge of medical terminology anatomy and physiology and English language rules to the transcription and proofreading of medical dictation from originators with various accents dialects and dictation styles.
Recognizes interprets and evaluates inconsistencies discrepancies and inaccuracies in medical dictation and appropriately edits revises and clarifies them without altering the meaning of the dictation or changing the dictators style.
Clarifies dictation which is unclear or incomplete seeking assistance as necessary.
Flags reports requiring the attention of the supervisor or dictator.
Uses reference materials appropriately and efficiently to facilitate the accuracy clarity and completeness of reports.
Meets quality and productivity standards and deadlines established by employer.
Verifies patient information for accuracy and completeness.
Formats reports according to established guidelines.
Understands and complies with policies and procedures related to medico legal matters including confidentiality amendment of medical records release of information patients rights medical records as legal evidence informed consent etc.
Recognizes and reports problems errors and discrepancies in dictation and patient records to appropriate manager.
Consults appropriate personnel regarding dictation which may be regarded as unprofessional frivolous insulting inflammatory or inappropriate.
Demonstrates an understanding of policies procedures and priorities seeking clarification as needed.
Maintains required records providing reports as scheduled and upon request.
Provides documentation of inservice and continuing education activities.
Reviews trends and developments in medicine English usage technology and transcription practices and shares knowledge with colleagues.
Documents new and revised terminology definitions styles and practices for reference and application.
Participates in the evaluation and selection of books publications and other reference materials.
Works and communicates in a positive and cooperative manner with management and supervisory staff medical staff coworkers
and other healthcare personnel and patients and their families when providing information and services seeking assistance and clarification and resolving problems.
Minimum education level of graduate degree or equivalent with work experience and continuing education.
A Minimum of 1 year of relevant experience in the concerned specialty in a Super/Multi Specialty hospital is desirable
Knowledge of medical terminology anatomy and physiology clinical medicine surgery diagnostic tests radiology pathology
Pharmacology and the various medical specialties as required in areas of responsibility.
Knowledge of medical transcription guidelines and practices.
Excellent written and oral communication skills including English usage grammar punctuation and style.
Ability to understand diverse accents and dialects and varying dictation styles.
Ability to use designated reference materials.
Ability to operate designated word processing dictation and transcription equipment and other equipment as specified.
Ability to work independently with minimal supervision.
Ability to work under pressure with time constraints.
Ability to concentrate.
Excellent listening skills.
Excellent eye hand and auditory coordination.
Certified medical transcriptionist (CMT) status preferred.
